REQUIREMENTS FOR APPLICATION PROCESS
To be considered for admission, the following steps must be taken:

1.

ADMISSION TO COCHISE COLLEGE
|
Be admitted as a student at Cochise College prior to submitting the application to the Cochise College Nursing Program. A letter from Cochise College Admission Office will be mailed to you indicating you have been accepted to the college. In this letter is a "C" number. This "C" number must be used on the nursing application form.

2.

APPLICATION SUBMISSION
DEADLINE: March 1, 2009

Submit application and waiver of licensure to the Cochise College Nursing Department.

3.
|
TRANSCRIPTS
DEADLINE: March 1, 2009
|
Official transcripts need to be submitted and are necessary to be considered for admission to the Cochise College Nursing Program. Applicants need to be aware that transcripts may exist under previous or maiden names.
Cochise College Transcript
To obtain a Cochise College transcript, write to the Cochise College TranscriptOffice and request that an “advising” copy of your Cochise College transcript be sent to the Cochise College Nursing Department. Telephone requests are not accepted. The address for the Cochise College TranscriptOffice is:

Other Colleges and Universities
To obtain transcripts from colleges other than Cochise College, you must request that an "official" transcript be sent to you. An official transcript will arrive in a sealed envelope. Do NOT open this envelope. Include this transcript with your nursing application form for submission.
If you have already had transcripts from other colleges or universities sent to Cochise College, please contact the transcript department and request a copy be send to the Cochise College Nursing Office.

4.

FINGERPRINT CLEARANCE CARD
DEADLINE: March 1, 2009

All applicants must obtain a fingerprint clearance card. The process to receive a Fingerprint Clearance Card is as follows:
STEP 1: Obtain a Fingerprint Clearance Card application (Form G) and a fingerprinting card (Form H) by contacting: Arizona Department of Public Safety: (602) 223-2279 You may also fax a request for an application to: (602) 223-2947
STEP 2: Take the blank fingerprint card to a local police department so that fingerprints can be properly and officially printed.
STEP 3: Mail the completed application, the inked fingerprint card, and the fee according to the directions listed on the application. It takes approximately 5 to 10 weeks for the application process.
STEP 4: After the application has been processed, you will receive the actual card back from the Arizona Department of Public Safety. This card is similar in appearance to a credit card. Submit a copy of the wallet-sized card with the nursing application.

5.

NET
(NURSING ENTRANCE TEST)
|
Once your application packet and all required accompanying documentation have been received by the Cochise College Nursing Department, you will be mailed information on the Nursing Entrance Test (NET) date(s), time(s), and location(s). The NET tests your basic comprehension of math and reading. The NET score that you receive is available for use for two years (the current year and the next year).
In order to be considered for admission, the applicant must achieve a national average score on the NET math and reading portion that is equal to or above the national average. The national average varies each year.
The Cochise College Nursing Department reserves the right to require minimum essential math skills and reading comprehension scores for admission to the nursing program.

6.

CALCULATION PROCESS OF ADMISSION SCORES
|
Each candidate whose NET composite percentage score meets the admission requirements will be considered for selection. Admission scores are calculated according to the following criteria:
■ You receive points for your composite percentage.
Example: If your composite score is 68, you receive 68 points.
■ You receive two (2) points for each documented, required, non-science, Associate Degree Nursing (ADN) general education course completed with a grade of “C” or better.
Example: English 101 = 2 points
■ You receive four (4) points for each documented, required, ADN science course completed within the last seven (7) years with a grade of “C” or better.
Example: Microbiology = 4 points
■ If you have been a legal resident of Cochise or Santa Cruz counties for one year, you receive five (5) points.

7.
|
ADMISSION TO THE NURSING PROGRAM
|
Admission to the Cochise College Nursing Program is decided by selecting the candidates with the highest number of total points until the class is filled. Qualified candidates for whom there is no place in the class at the time of first selection will be placed on an alternate list and notified if a space becomes available. Alternate students will not be accepted after July 1, of the application year even if there are subsequent student withdrawals.
Those candidates (including alternates) not accepted for the current year must submit a new application for the next year. Applications are maintained in the Cochise College Nursing Department until July 1 of the current application year. NET results are maintained in the Cochise College Nursing Department only if a written request has been provided to maintain your file for the next year.

8.

LPNs (or LVNs) WITH A CURRENT LICENSE

LPN Challenge Process
|
An LPN with a current license may be able to challenge the first year of the Nursing Program and enter directly into the second year. To do so, the applicant must meet certain criteria (Form F) and take a series of challenge tests. 9.

TRANSFERRING NURSING STUDENT
|
Applicants who wish to be considered for advance placement from an accredited nursing program must contact the Cochise College Nursing Department for details and deadlines. Please contact the Cochise College Nursing Department at (520) 515-5309 or (520) 417-4016 for challenge details and deadlines.

10.
|
APPLICANT FILE
|
It is the responsibility of the student to contact the Cochise College Nursing Department whenever any of the following has occurred at any time during the application period:
■ Change of address
■ Change in phone number
■ Change in e-mail address
It is important that the Cochise College Nursing Department is contacted directly when any of these changes occur, even if you have already informed other Cochise College departments.
