Cochise College Foundation - Forms

The following forms are to be used internally.

Requesting Funds from the Cochise College Foundation
Departments seeking to make purchases using foundation funds designated for specific programs or purposes may use the following forms.

Foundation Transfer Request
Use this form to make purchases that are permissible on college funds. This form is not to be used to award scholarships. Contact the Cochise College Foundation to ensure funds designated for the purchase are available. Requests should be made in advance of purchase. Once all signatures and forms are received, the foundation will provide a check to the college, for deposit in the appropriate account, and the requesting department will follow standard college purchasing procedures.

Foundation Check Request
Use this form to make purchases that are not permissible on college funds. Contact the Cochise College Foundation to ensure funds desigated for the purchase are available.

Requesting New Accounts be Established
with the Cochise College Foundation

Departments seeking to establish a new account with the Cochise College Foundation should document the request using the following form. New accounts may be considered by the Cochise College Foundation board once the requestor has identified possible contributors, determined the type of account and who will be able to access it, and obtained the appropriate signatures.
